• Please hand in your forms with fee in person or by mail.  We don't accept faxed in or emailed registration forms. 

• DON'T mail cash with your registration form. Make cheque payable to “AFC” and make sure you have enough money in your account. NSF penalty is $20.

• For mail-in registration, Confirmation Receipt will be sent through email or fax or regular mail before the conference (please write your email address clearly).  If you mail your form in and do not have an email address or fax, please provide us with an envelope with a stamp and address, so that we can mail the receipt back to you.  If you don't receive the confirmation by 2 weeks before the conference, please contact us.

• Limited Admission!  First come first served. Registration will close as soon as the conference is full or on March 9, 2005.  No registration at the door.  

• Refund Policy - If you couldn't make it to the conference after you registered, please inform us a.s.a.p. and we'll give you back your registration fee (-$10 administration fee, no later than March 9, 2005).  No transfer of space (I.D. will be checked at the conference).

• Please bring your CONFIRMATION RECEIPT and HEALTH CARD (or photocopy) to the conference.  (We need your receipt for registration and your health card for emergency.)

• To guarantee for a TC T-shirt, please check the appropriate box in the registration form and pay $10 with your registration fee.
